Uninsured Services Fees
Our clinic is in compliance with the AMA guidelines to billing Uninsured Services.
Fees listed below are the patient’s responsibility. They are not covered by Alberta Health Care or Blue Cross benefits.
Your physician or an assistant will discuss any fee with you before providing an uninsured service.
Payment is expected when you receive the service. We accept cash, cheques, debit or credit cards.
MEDICALS:
Driver's Medical (Class 1-4) $ 150
Driver's Medical (Class 5 or 6) $ 75
ECG $ 25
Life Insurance $ 100 & up
Life Insurance with ECG $ 125
Capacity Assessment Reports $ 200
Non-resident of Canada Doctor Visit $ 120
Pilot's Medical $ 150
Pilot's (with Driver's Medical OR ECG) $ 175
Pilot's (with Driver's Medical & ECG) $ 200
Pre-admission Medical Assessment (fitness to work forms) $ 100
REPORTS AND FORMS:
Completion of forms (minimum fee) $ 55

Senior Resident $ 30
